Social Care

The Integrated Children’s Disability Service was created in September 2016 and brings together education and social care into one team to deliver a holistic approach to support children and young people with disabilities aged 0-25 years.

They offer families access to:

  • after school and holiday clubs
  • sitters and befrienders
  • support for parents
  • activity groups for other children in your family
  • short breaks for disabled children

To be eligible for support you first need to complete a care and support assessment.

Assessment Team

The Assessment Team, receives requests for Short Breaks, Specialist childcare packages and Education, Health & Care (EHC) Needs Assessments via the Disability Duty point in relation to children, young people (CYP) with special educational needs & disabilities (SEND). The Assessment Team assesses their eligibility in relation to these requests, and if eligible progresses them to ensure their identified outcomes are met and if not signposts them to other services. The Assessment Team works closely with other services in relation to identifying families who may need support with day-to-day routines, behaviour management, sleep routines and families who may benefit from a multi-agency approach.

Commissioning & Contracts Team

The Commissioning Team commissions education placements for children and young people with an EHCP (Education Health & Care Plan) who cannot access mainstream or special school education. Post 16 provision is commissioned for children & young people who require additional support to access the curriculum.

We also commission packages of support for children & young people assessed as eligible for Short Breaks provision to meet their identified outcomes and appropriate childcare packages for those children & young people unable to access universal/mainstream childcare providers to enable parent/ carers to work.

Occupational Therapy

The OT Service provides specialist assessment for disabled children and young people resident in Nottinghamshire up to the age of 18 who have a disability which has a substantial adverse effect on their ability to carry out normal day to day activities.

They can provide the following equipment on short term loan that can be used for holidays, family trips, social events or at school:

  • Molift folding mobile hoists
  • Manual transfer slings
  • Portable safe spaces
  • Maclaren Major Elite buggies
  • Early sitting systems
  • Stairclimbers
  • Temporary ramps
  • Toileting and bathing equipment
